End of Life Planning with Advance Directives & POLST

An Advance Directive is a tool which can help an individual communicate personal choices about end of life care if the person becomes unable to comprehend a certain situation or verbalize a choice at the time a decision is needed.  POLST stands for Physician’s Orders for Life Sustaining Treatment.   Learn how these two documents differ and how they can help you create a peaceful end of life plan that is consistent with your values.  Just as you plan for milestones during your lifetime such as retirement, it is important to plan your goals for the end of your life in advance.  The more decisions you can make in advance, the easier it will be for your loved ones when the end of life approaches.

This seminar will be presented by Attorney Ryan Webber and is free.  No registration is required.
