FREE Health Care Power of Attorney Documents

Everyone is looking for a way to help as we face this public health emergency together. We at Keystone Elder Law want to support our community in an important way. Many people delay making important legal plans. Some planning is necessary in case a person gets sick and cannot communicate with a doctor about treatment decisions. That’s where we come in.
In response to the COVID-19 pandemic, we will be offering free Health Care Power of Attorney/Advance Directive documents to residents of Pennsylvania age 18 and older. Our Health Care Power of Attorney document allows you to appoint another individual as your agent to act on your behalf if you become unable to make medical decisions for yourself. It also allows you to document your wishes related to certain types of medical care. This document is fully compliant with Pennsylvania law. You will be able to obtain a document from the safety of your own home.
This service is offered in support of health care providers in hospitals, nursing homes, and other health care facilities/practices; first responders; grocery store and pharmacy employees; and employees of other essential businesses who face an increased risk to their own health due to this virus.
